Procedures For Requesting Disability Services
Step 1: As early as possible (but no later than three weeks before any instruction begins), contact us:
UCSB Extension (Attn. Disability Services Representative)
Mail Code 1110
University of California, Santa Barbara
Santa Barbara, CA 93106-1110
Ph: (805) 893-2944*
Fax: (805) 893-4943
*You may also use the California Relay Service (CRS) by calling (800) 735-2929 TDD or Voice; or (800) 735-2922 Voice to TDD (this relays telephone calls between TDD and voice callers).
At this time, the representative will discuss your individual needs and will set up an appointment to meet with you within 7–10 days.
Note: Requests to meet with a disability services representative that are received later than two weeks before instruction begins may limit our ability to provide you with the requested services in time for the first day of class.
Step 2: Send your disability services representative the following items in time for the meeting:
- A written request outlining your needs and describing your limitations due to disability as covered under the Americans with Disabilities Act of 1990 or Section 504 of the Rehabilitation Act of 1973.
- Written notification on official letterhead from a physician, certified specialist, professional counselor, or appropriate school official verifying your disability and outlining any need for special services. Depending on your needs, additional documentation may be requested.
Step 3: Meet with your disability services representative. At the meeting, your request will be confirmed, a plan will be determined for providing you with services, and a timeline will be established for when services will begin. If you have not already submitted the items requested in Step 2, please bring them with you to your meeting. Services will only be arranged once you provide payment in full for UCSB Extension course(s).
If any of our materials on accessing disability services are not accessible to you, please contact our department (see contact information above) and we will provide alternatives.
